GRACE – bridGing gaps in caRdiAC health managEment

Innovative public-private partnerships and essential knowledge can enhance healthcare services for cardiovascular disease. By leveraging advanced technologies, these partnerships can identify and address current barriers, ensuring a seamless continuum of care and optimised pathways with GRACE’s innovative solution. The EU-funded GRACE project will improve the clinical pathway for CVD by adopting a holistic view of patients and enhancing healthcare services with advanced technology.
