Abstract
What to eat today so as not to get sick tomorrow? In the age of evidence-based medicine, a lot of confusion persists in recommending healthy diets. It is repeated that large, long-term randomized trials in nutrition are lacking. Observational studies are thought to support statistical correlations, but not causality. But almost everything we know is based on these observational studies. The exception is the Mediterranean diet, which has been established as the paradigm of a cardio-healthy diet thanks to a large randomized trial (PREDIMED) that demonstrated a 30% reduction in the risk of stroke, heart attack or cardiovascular death. But is PREDIMED an exception that will not be repeated? To what extent are these types of trials ethical and feasible? Are purely observational studies any use? What alternatives are more valid to base the nutritional recommendations given to the population? What is known for sure?
Addressed: Cross-cutting to all areas. Of special interest to groups in area D - Endocrine Diseases and Nutrition, as well as primary care groups.
